Case Coordination

FCP’s Case Coordinators work with you to identify linguistically and culturally appropriate home attendant placements. They develop schedules, provide direction on the plan of care, and communicate your preferences for all your home care needs. The Case Coordinator works with the nursing staff to ensure that your home health care needs are implemented. Should you have any concerns or issues with your home care, the Case Coordinator is there to assist you in every way.
